Individual & Group Case Management
Case Management is the strategic process of individual goal achievement. Case Management is a tool that youth can utilize here at the PYFC to move forward on achieving personal goals, and moving closer to individual accountability and responsibility. The Case Manager would be used as a navigator, resource, mentor, and an accountability partner through the attainment of these individual goals set by the youth that choose to participate.
